HARD TO FIND!
This is a small, pattern #905, Wreath & Shell (OMN Manila) Vaseline Opal Toothpick Holder made by Model Flint Glass Co (Albany, IN). It is a difficult item to find. In 1900 this factory was part of the National Glass conglomerate.
This sits flat on 3 feet. There are a lot of items in this pattern & color; you can have a good collection in this pattern.
I use a small 5 LED UV Flashlight for the Blacklight photo(s).
